This behavior is expected. Sometimes, the file source information is not available for DLP Endpoint to capture. 

Example
If you save a Microsoft Office document directly to a removable storage device using Save As, some incidents might have a source file location and others might not. 
The available source data is inconsistent. So the product development team made the design decision to provide consistent file destination information instead.
